Making Requests for Disclosure or Correction of Personal Data Held

Any requests for disclosure or correction of customers' personal data held by Bank of America, National Association, Tokyo Branch (the “Branch”) following the Personal Information Protection Law shall be in accordance with such procedures as the Branch designates including a confirmation of identification of the "Principal" of the said personal data, and completion of a request form described below.

1. Contact

Please contact Compliance (phone: 03-6225-8093).

2. Request Method

Please fill out all items in the "Personal Information Disclosure and Correction Request Form," seal or sign it, and submit it to Compliance.

3. Confirmation of Identification of the Principal of Personal Data

For customers who do not have an account with the Branch

Identification of the Principal will be confirmed from the signature or the registered seal attached on the "Personal Information Disclosure and Correction Request Form."

 

For customers who do not have an account with the Branch

Documents to confirm identification of the Principal shall be submitted to the Branch together with the "Personal Information Disclosure and Correction Request Form."

4. Charges for Requests and Payment Method

For each request for disclosure or correction, a standard fee established by the Branch will be charged.

 

For customers who have accounts with the Branch, such fees will be withdrawn against the balance of the customer's account. For customers who do not have an account with the Branch, payment of such fees shall be transferred to a bank account designated by the Branch.

5. Items in Request Form

Relevant items for disclosure or correction of personal data, such as reason for request, period, name of department or office handling the said personal data, term, etc., shall be entered in full in the "Personal Information Disclosure and Correction Request Form."

6. Reply from the Branch

Upon receiving the fully completed "Personal Information Disclosure and Correction Request Form" and payment of the requested fees, the Branch will promptly send a reply by mail to the customer's forwarding address.
